I am trying to install IRIX 6.5.15 on an Indigo2 and whenever I try and
start up off of the CD I get:

Exception: <vector=UTLB Miss>

What kind of cd-rom do you use (model, speed)?
Have you checked scsi cables + termination?
What is your boot command?

What kind of cd-rom do you use (model, speed)?

The cd-rom is a Toshiba Model No. XM-5401B, Ver. No. 005, ROM Ver.
MD60212. I'm not sure how to tell what speed it is. I think the SGI
part number is 064-0033-001. I am pretty sure it is not a scsi cable
of termination problem because I can get into the fx utility on the CD
by issuing the following command at the monitor:
boot -f dksc(0,3,8)sashARCS dksc(0,3,7)stand/fx.ARCS --x

I boot by entering 2 at the prompt to Install System Software.
Here is the hinv:
System: IP22
Processor: 200 Mhz R4400, with FPU
Primary I-cache size: 16 Kbytes
Primary D-cache size: 16 Kbytes
Secondary cache size: 1024 Kbytes
Memory size: 384 Mbytes
SCSI Disk: scsi(0)disk(2)
SCSI CDROM: scsi(0)cdrom(3)
Audio: Iris Audio Processor: version A2 revision

Please check if there is _defintely_ a terminator attached. Without
terminator it could be possibe to boot, but when copying mini root from
CD it fails (own experience, same CD drive)

btw: I recommend an active terminator.

is there a specific reason for using LU 2 for the disk? Did you
set ...disk(2)... in SystemPartition and OSLoadPartition accordingly? I
dont remember whether IRIX installation sets these automatically
according to the disk LU, it's been such a long time since I owned an
Indigo 2

So you are using the drive sleds for the disk and the CDROM. Did you set
any termination jumpers on either of these? IIRC you should not since
termination is handled by the Indigo2 internally.

Have you burned cd-rom by your self or have you original cd-rom?

If you have made it, i recommend you that you burn that cd again at 2x
or 4x speed. And boot it with:

boot -f dksc(0,3,8)/stand/sash dksc(0,3,7)/stand/fx.ARCS --x
then
boot -f dksc(0,3,8)/stand/sash (and install miniroot from there)

Just wondering if you're using the right boot command. The ARCS is
ok for R4K, but wrong for R8K/R10K.

Also, what is the output from printenv? Make sure your system disk is
on
SCSI ID 1 and also make sure that the various PROM settings match
accordingly. To be sure, enter this in Command Monitor:

resetenv

If your system disk is in the upper bay, then move it to the lower bay.

As others have said, make sure the CDROM has parity ON and, if
still problems are seen, termination ON. If block size is an optional
feature of the CDROM, make sure it is set to 512.

Most of us probably have in mind external cd-rom and there proper
termination is required.

Why should be system disk at ID 1? As i know system disk can be at every
ID except controller ID 0, but you have adjust prom enviroment accordingly.

in another pposting on this thread, the OP quotes a hinv of his I2.
You'll see there that he has the disk at LU 2 and the CDROM at LU 3,
both on the internal SCSI
